What is pictured in 1st picture has no options and will look like that with default selections made.
Mount can be installed horizontally as well as vertically to fit in very narrow situations.

Swivel has a hole to allow wires to pass through the back. Screws are included to mount and secure the perfect angle so It doesn’t move when bell is pressed.
